Jenny tells us about her work as a community matron in Stoke and how the role differs from that of a GP.

Her goal is to reduce the number of people with long-term health conditions being admitted to hospital.

She works at spotting early symptoms and then deciding which is the best treatment pathway for the individual patient.

This reduces strain on A&E departments and saves money but more importantly it improves the quality of life for patients with long-term conditions.

Note (6 March 2012): Since making this film in 2010, Jenny has become an advanced practitioner in the single point of care for Staffordshire and Stoke-on-Trent Partnership NHS Trust.
