More than one million people in the West Midlands have at least one long term condition and this number continues to rise year after year.

In this film NHS local hear from NHS West Midlands' Jo Harding, the programme lead for long term conditions, and Jane Diggle, senior project manager.  

A new approach to collaborative care has begun in which patients with long term conditions are given the opportunity to discuss treatment with the clinician. The patient leads the consultation about their health and social requirements. They work together to create a personalised care plan.

The personalised care plan is tailored to the patients needs. The patient is involved every step of the way and goes away with a document clearly setting out exactly what has been put in place which they can refer to and share with those around them.
